Medical History

Admitting Diagnosis

The 64-year female patient had severe headache and unconsciousness 7 hours before administration. The CT and CTA examination showed subarachnoid hemorrhage and aneurysm at the anterior communicating artery.
The patient had hypertension in the past.
The patient was unconscious and could only voice but not speech. The size of both pupils is equal, ca. 3.0 mm in diameter. Sensitive to light. Normal superficial sensation, normal deep sensation, no pathological sign.

Aneurysm at the anterior communicating artery, SAH. 

Operation Plan

Stent-assisted coiling embolization.

Operational Procedures

1. Using 8F guiding and 125cm catheter surpassed to the C1-segment of the right internal carotid artery. Deliver the 6F 115cm Skathi™ Distal Access Catheter to the C4-segment of the right internal carotid artery.

2. Deliver the Stent system to the parent artery and the coil to the aneurysm.
3. Perform embolization with the coils (Axium 3mm*8mm, Cosmos 2.5mm*4mm, Target 2mm*4mm, Helical 1.5mm*3mm, and 1.5mm*2mm) with the assistance of Neuroform Altas 3mm*1.5mm.

Comments and summary of the Surgeon

1. The carotid artery and internal carotid arteries of this patient was tortuous. The 6F 115cm Skathi™ (Minjie) Distal Access Catheter may provide support at both proximal and distal level for device delivery. It shall be mentioned that it would be even better to choose 125cm Skathi™ to access a higher level.

2. The Skathi™ (Minjie) Distal Access Catheter has a flexible shaft to easily pass through the tortuosity and access to a high level. The lumen allows passing double microcatheters with good kink resistance and visibility.
